Therefore, if you are going to become our patient or you plan to continue to be our patient, you need to contact us BEFORE you sign up with an advantage plan or BEFORE you change to a different plan. Some of the plans have restrictions that do not allow us to provide the level of care that we expect to provide for our patients. We cannot jump through their hoops and still be denied basic requests for care for patients.Â
Please understand that some of these plans auto-assign us to all of their plans whether we have signed up for each plan or not, so just because they tell you that we are "in-network" does not mean that we will be accepting that plan. Please call our office to verify what plans we are currently accepting.Â

If you have been exposed to someone with a positive COVID diagnosis or you are having symptoms of COVID (fever, sore throat, shortness of breath or difficulty breathing, cough, headache, loss of taste or smell, muscle or body aches, fatigue , diarrhea or nausea and vomiting), please notify the person scheduling your appointment or the person that you speak to when you check in for your appointment. Our staff is doing everything possible to protect you and others from the spread of this virus.
We also ask that during this time, only the patient having the appointment come in for the visit unless you are under the age of 18 or you need special assistance.
